Subject: Availability of the New Bureau of Land Management Form 5450-5
Purpose: This Instruction Memorandum (IM) announces the availability of the updated Bureau of Land Management (BLM) Form 5450-5, VEGETATIVE MATERIAL NEGOTIATED CASH SALE CONTRACT ($2,499 or less for vegetative material).Program Area: Forest, Range, Riparian and Plant Conservation.
Policy/Action: The BLM Form 5450-5 for sales of vegetative materials from designated BLM-managed lands has been revised. The form retains the same form number, all mineral related information has been removed and minor clarifications made. The revised form supersedes all previous versions of Form 5450-5, Vegetative or Mineral Material Negotiated Cash Sale Contract ($2,499 or less for vegetative materials) ($2,000 or less for mineral materials) for sales of vegetative materials.

All requests must contain the following information: name, physical street address, and phone number. The revised BLM Form 5450-5 will continue to be issued in books that contain 25 contracts; each contract will have one original and three carbon copies for records. Do not use any previous versions of Form 5450-5 and destroy all remaining inventory.
Instructions are included with each book of forms for issuing the contracts. It is important that the authorized officer clearly identifies the designated areas, materials, and quantity of vegetation materials authorized for removal. Entry of information from the forms into the Collection and Billing Financial System (CBS) and the Special Forest Products (SFP) databases is required per the BLM policy.
Timeframe: Effective immediately.
Budget Impact: None.
Background: The previous dual purpose Form 5450-5 authorized the removal of both vegetative and mineral materials. The newly revised Form 5450-5 authorizes the removal of vegetative materials only. For sale of mineral materials, please use Form 3603-10 MINERAL MATERIAL NONEXCLUSIVE CASH SALE CONTRACT (See IM No. 2014-152). The BLM uses the Form 5450-5 to document and authorize sales of vegetative materials from designated BLM-managed lands.
Manual/Handbook Sections Affected: The current policy in BLM H-5000-1 and H-5400-1 addresses appropriate use of this from. The BLM will incorporate any additional policy and instruction as necessary during the next revision.
Coordination: The Division of Forestry, Range, Riparian and Plant Conservation (WO-220) coordinated development of this IM with the BLM offices, the mineral materials programs (WO-320), the National Support Programs Section (OC-652), and the Washington Office Division of Budget.
