Subject:           Availability of Bureau of Land Management  Forms 4130-1, 4130-1a, 4130-1b, 4130-3a, 4130-4, 4130-5

Program Area:  Grazing Administration

Purpose:  This Instruction Memorandum announces that the Office of Management and Budget (OMB) has approved use of six Bureau of Land Management (BLM) grazing program administration forms until August 31, 2014, and directs field officials to destroy previous editions.

Policy/Action:  Existing forms 4130-1 -- Grazing Schedule-Grazing Application; 4130-1a -- Grazing Preference Application & Preference Transfer Application; 4130-1b -- Grazing Application-Supplemental Information; 4130-4 -- Application for Exchange-of-Use Grazing Agreement; and 4130-5 -- Actual Grazing Use Report, now have a February 2012 edition date and an August 31, 2014 OMB expiration date.  Destroy all previous editions of these forms. 

These forms will not be stocked at the Printed Materials Distribution Center (PMDS).  The public and BLM staff can locate these forms at the BLM’s Form Central website (https://www.blm.gov/noc/st/en/business/eForms.html).  The BLM staff may also access them at BLM’s internal forms website (http://web.blm.gov/blmforms/).  The forms are in fillable pdf format. 

Form 4130-3a, Grazing Application-Grazing Schedule is generated by BLM staff from the BLM’s Rangeland Administration System (RAS) for program purposes.  It also now has a February 2012 edition date and an August 31, 2014 OMB Expiration Date.

Timeframe:  Effective immediately. 

Budget Impact: Not maintaining these forms at PMDS reduces BLM costs for form printing, stocking and shipping.

Background:  The BLM uses these forms to collect information required to process applications for grazing preference transfer, grazing permits and leases, exchange of use authorizations, and to document actual use information (see 43 CFR Part 4100).  The BLM has used these or similar forms to collect this type of information for the last several decades.

Coordination Occurred Among:  Division of Forest, Rangeland, Riparian and Plant Conservation (WO-220), RAS User Representative (OC-580), and Bureau Forms Manager (OC-652).
