Oil & Gas Inspection Forms


Welcome to the BLM website for oil and gas inspection forms. This website provides an overview and access to blank inspection forms used to document field inspections.
 
Surface Drilling and Inspection Form is used primarily by our natural resources, surface and environmental protection staff to document surface inspections during construction and immediately after construction of well/facility locations, roads, pipelines, power lines, and other surface disturbing operations.
 
Surface Production and Interim Reclamation Inspection and Monitoring Form is used primarily by our natural resources, environmental and surface protection staff to document surface interim reclamation and monitoring inspections of well/facility locations, roads, pipelines, power lines, and other surface disturbing operations.
 
Reclamation Inspection Monitoring Form is used primarily by our natural resources, surface and environmental protection staff to document surface final reclamation and monitoring inspections of well/facility locations, roads, pipelines, power lines, and other surface disturbing operations.
 
Drilling Inspection Form 3160-10 is used by our Petroleum Engineering Technicians/Oil and Gas Inspectors and Petroleum Engineers to inspect drilling operations beginning when a well is spudded and till the final depth is reach and the drilling rig is removed from location.
 
Production Inspection Form 3160-11 is used by our Petroleum Engineering Technicians/Oil and Gas Inspectors, Production Accountability Technicians, and Petroleum Engineers to inspect producing, injection and disposal wells and facilities.
 
Abandonment Inspection Form 3160-13 is used by our Petroleum Engineering Technicians/Oil and Gas Inspectors and Petroleum Engineers when witnessing plugging operations for well or formation abandonment.
 
Gas Measurement Form 3160-15 is used by our Petroleum Engineering Technicians/Oil and Gas Inspectors and Petroleum Engineers when witnessing gas meter calibrations, orifice plate inspections, and gas sample collections.
 
Tank Gauging Form 3160.16 is used by our Petroleum Engineering Technicians/Oil and Gas Inspectors and Petroleum Engineers when witnessing tank gauging operations. 

LACT Form 3160-17 is used by our Petroleum Engineering Technicians/Oil and Gas Inspectors and Petroleum Engineers when witnessing Lease Automatic Custody Transfer (LACT) meter provings.

Onshore Oil and Gas Order No. 3 requires operators to self-inspect their production facilities.  In addition to the required self-inspection, the BLM encourages operators to self-inspect all of their wells, facilities, and operations.  The operators can utilize the inspection forms above as a guide for conducting the inspections.  The BLM also encourages operators to implement and self-inspect for Best Management Practices and the operating standards and guidelines in the Gold Book.
