Subject: Availability of Bureau Form 3160-21
This Information Bulletin (IB) identifies the availability of a new Notification Of Well Spud Form for use in accordance with regulations 43 CFR 3160. Use of this form is optional.
The Bureau of Land Management (BLM) form listed below, that relates to fluid mineral operations and surface management activities under regulations 43 CFR 3160, is optional and internal and as such does not need to have approval from the Office of Management and Budget.

This form is used by the BLM to collect information in managing surface disturbing activities carried out during oil and gas operations under regulations 43 CFR 3160.
