Searching for a Form
The Search feature in Forms Central allows you to search by Title, Keyword, Form Number, or OMB Number.
Guidelines for search terms:
- Searches are not case-sensitive: “APPLY” is the same as “apply”
- Do not use wildcards or special characters such as asterisks(*), slashes (/), dashes(-) or apostrophes (‘).

Search results will return all matches, including those characters found within other
words, without the use of any wildcards. or example, a search for “form” will
return both the following links:
“Volunteer Form” and “Information Request”.
Opening a Form
All forms are available in one or more of the following formats: SP, Fillable PDF, Static PDF.
To open a form from Forms Central:
- Find the form by searching or navigating the folders.
- Click on the form number or title.
- Information about the form will be displayed including "Available Formats". Click on one of the available
formats to open the form. For more information on using each of the formats, return to the Forms page.
